Pathways to Medication Support Services

At Possibilities, we understand that clarity, confidence, and care must accompany every step of your journey—assessment, therapy, skill‑building, and when appropriate, medication. Our clinicians, physicians, and nurse practitioners work in concert with you so that medication decisions are evidence‑informed and personalized, based on your real‑world experience. Below you’ll find three flexible pathways to medication support.
Our Nurse-Practitioner Titration and Follow-Up Service is available to clients who have completed any Possibilities Assessment with an ADHD component. Our Focus Forward 360 ADHD Assessment, an ADHD Direct Assessment, a Signature Psychoeducational Assessment or an Early Years Assessment where ADHD has been diagnosed would all qualify. Following an assessment where ADHD has been diagnosed, the Possibilities Assessment Team will create a Medication Treatment Plan. You may choose to have a Nurse Practitioner (NP) prescribe, adjust, and monitor the medication over time within our clinic based on this plan.
How it works
● Nurse Practitioner (NP) appointments (30 minutes) focus on how you or your child respond to the prescribed medication and each dosage change.
● Your NP documents feedback in detail and adjusts the Medication Treatment Plan as necessary.
● Appointment frequency is set collaboratively to match your needs.
Fees and coverage
● NP visits are $100 per session (NPs are not covered by OHIP).
● Some extended‑health plans reimburse nursing services. Please check with your insurer.
Following your assessment at Possibilities Clinic where ADHD was assessed and diagnosed, the Possibilities Clinic’s Physician on your Assessment Team will send the Medication Treatment Plan to your Primary Care Provider. Your Community Physician or Nurse Practitioner will then prescribe, titrate, and monitor the ADHD medication treatment. If any questions or concerns arise during this process, your Primary Care Provider can reach out to the Possibilities Clinic Physicians for discussion and consultation.
If you do not wish to pursue an assessment at Possibilities, you can still get medication advice from our clinic. Your Community Physician or Nurse Practitioner can reach out to our staff at Possibilities through an OHIP-covered eConsult to answer questions about your treatment. You will not be seen directly by our staff with this option, but our staff will be able to answer your Primary Care Provider’s questions and help guide their care.
