We can support eligible forms related to government funding, disability supports, and accommodations. This includes DTCC (Disability Tax Credit Certification) forms and other government-related documentation. The specific forms we can assist with depend on your individual needs and program requirements.
Most appointments are focused and efficient, typically taking 50 minutes to one hour. The exact time depends on the complexity of your form and the information needed.
The clinician will prepare and complete appropriate portions of the form within their scope of practice. When a physician assessment, review, or signature is required, we coordinate with the appropriate physician to ensure those sections are completed.

The $400 + HST fee is an out-of-pocket cost for this specialized service. Some private insurance plans may cover portions of clinician services—we recommend checking your coverage. We can provide a detailed receipt for insurance claims.
